UT Health Austin and The Office of Research at Dell Medical School work collaboratively to develop and maintain operational resources and documentation for conducting research at a UT Health Austin location and/or with UT Health Austin patients.

This research project intake form enables the conduct of research-related activity at UT Health Austin and/or interaction with UT Health Austin patients, through the appropriate operational and financial processes for shared resources.
See instructions on obtaining access to UT Health Austin Clinics or systems for faculty/staff of Dell Medical School as well as faculty/staff employed through another UT Austin School or non-UT Austin entity, students, or unaffiliated research volunteers.
Athena is UT Health Austin's electronic medical record system. Athena Documentation Guidelines to be used when there is one or more of the following:
- a new research coordinator who needs access to Athena,
- a new research study that needs to be set up in Athena,
- a UT Health Austin patient who needs to be flagged as a research participant, and/or on when/how to enter research-related clinical services for billing
Research Informed Consent Documents
- Complete cover sheet and fax consent to UTHA Health Information Management (medical records)
- Applies to all research participants who are UT Health Austin patients
Submit a data request here. This form is to be used by principal investigators at Dell Medical School who are requesting data on behalf of an approved research project.
Once submitted, the review may include consultation with the Dell Medical School Office of Research and the Health Transformation Research Institute.
